The Health Zone Nurse Practitioner-Led Clinic (HZ NPLC) is looking for an experienced NP Leader with a passion for clinical practice and the growth and continued development of the NPLC model of care in Ontario. Situated in beautiful London, Ontario, the HZ NPLC was established in 2011 and moved to its modern, bright clinic space in 2021. Dedicated to improving health and well-being, the clinic delivers comprehensive primary care, health promotion, disease prevention, chronic disease management, and care coordination within a multidisciplinary team. The HZ NPLC focuses on serving individuals and families who may have previously lacked access to a primary care provider, ensuring they receive coordinated and continuous healthcare
Functioning within a co-leadership model, the NP Lead will work in partnership with the Administration Lead and will be responsible for the effective and efficient operational running of the clinic in support of equitable clinical outcomes.

Reporting to the Board of Directors and holding accountability to Ontario Health, the NP Lead’s key accountabilities include:
- Providing primary care for a defined patient panel:
o The NP role is dynamic, shaped by leadership and administrative expectations, with approximately 0.6 FTE dedicated to clinical responsibilities, managing a patient panel of 500-600 patients.
o Serves as a Most Responsible Provider (MRP), delivering full-scope primary care.
- Board Relations & Strategic Leadership:
o Maintains strong relations with the Board of Directors, providing regular updates and reports on clinical operations and the strategic plan
o Enacts strategic leadership to advance the clinic’s goals
o Implements the vision, mission, values and strategic plan
- Human Resources & Team Leadership:
o Assists with the recruitment, selection, supervision and evaluation of interprofessional staff (13 FTE, non-unionized)
o Supervises mentors, orients, and precepts professional employees, including other NPs
o Conducts performance appraisals
o Promotes strong team development and fosters a positive, productive workplace culture
o Coordinates regular NP and multidisciplinary team meetings
- Policy, Compliance & Policy Management:
o Develops, implements, and ensures adherence to policies and procedures in conjunction with the Board of Directors
o Oversees quality management initiatives to enhance patient care and operational efficiency
- Collaborative Partnerships:
o Establishes and maintains partnerships with community service organizations, agencies, hospitals and educational institutions to support primary healthcare networks and reciprocal referral relationships.
o Develops collaborative relationships with local NPs, Family Health Teams and Community Health Centres, Middlesex London Ontario Health Team (MLOHT), NPLCA, NPAO, Alliance for Healthy Communities, Ontario Health, and the MOH
- Communications & Community Engagement:
o Leads internal and external communications, including social media and outreach efforts
o Strengthens public, community, and political advocacy efforts
- Operational Accountability:
o Supports and participates in effective and efficient clinic operations in collaboration with the Administration Lead, ensuring fiscal responsibility and accountability to the Board of Directors and Ontario Health.
o Assists in managing information technology to enhance clinical and operational efficiency, with primary responsibility held by the Administration Lead.

Salary Range: $130,000–$135,000 a year
Job type: Permanent, Full-time
Hours: Monday to Friday, with some evening/extended hours required
Qualifications
- Successful completion of a Master’s Degree in Nursing
- Current registration in the extended class in Primary Care and in good standing with the College of Nurses of Ontario
- Current member of the Nurse Practitioner’s Association of Ontario (NPAO)
- Mandatory Professional Liability Protection and Liability Insurance
- Minimum 3-5 years demonstrated effective leadership and management experience in a community, hospital or primary care setting; preference will be given to those with additional skills in Finance and Human Resources management
- Minimum 5 years’ experience in primary care or an acute care environment in an NP role
- Satisfactory criminal reference check, current BCLS, and valid Ontario driver’s license
- Experience in a variety of clinical environments and knowledge of provincial health policy
- Visionary leader who is committed to the principles of Diversity, Equity and Inclusion and ensuring equitable services to patients who may be vulnerable and marginalized
- Experience or training in providing culturally safe care for Indigenous patients and communities, and other marginalized populations.
- Understanding of trauma-informed principles in patient care and staff leadership.
- Strong written and verbal communication skills; additional language proficiency considered an asset
- Proven ability to lead organizational change and adapt to evolving healthcare policies and regulations.
- Ability to analyze healthcare data to drive evidence-informed decision-making and quality improvement initiatives
- Familiar with healthcare regulations, risk mitigation strategies, and accreditation processes.
- Experience managing workplace conflicts and fostering a positive, productive team environment.
- Demonstrated skill in insight, self-awareness and self-management consistent with high emotional intelligence
- Demonstrated political advocacy
- Experience with electronic medical records system and Ontario Drug Formulary; experience with Telus PS Suites is an asset
- Computer proficiency with Internet, Microsoft Office 365 including Outlook, Automated Clinical Systems and Virtual meeting platforms
- Demonstrated experience in education and program development and eligible for an Adjunct appointment to an academic institution
- Proven ability to maintain regular attendance while fulfilling leadership responsibilities in a collaborative team environment.
