What is a Nurse Practitioner Led Clinic (NPLC)?
Nurse Practitioner Led clinics are an innovative model for delivery of comprehensive primary health care in Ontario and Canada. The model is designed to improve access to care for the thousands of individuals and families who do not currently have a primary health-care provider. One of the unique aspects of the model is the incorporation of nursing leadership within an interprofessional team. The clinics improve the quality of care through enhanced health promotion, disease prevention, mental health care and chronic disease management, as well as improve care coordination and navigation of the health care system. Working as partners with patients in their care plan is an important aspect of the underlying philosophy of NPLCs. The team includes Nurse Practitioners, Registered Nurses, Registered Social Workers, a Community Health Promoter, Medical Office Assistants, and Administrators. We also have two consulting doctors who we discuss and plan care with when needed.
What is a Nurse Practitioner (NP)?
A nurse practitioner (NP) is a registered nurse with advanced university education who provides personalized, quality health care to patients. Ontario nurse practitioners provide a full range of health care services to individuals, families and communities in a variety of settings including hospitals and community based clinics in cities and smaller towns in Ontario. We work in partnership with other health care professionals to keep you, your family and your community well.
Registered Nurses in the Extended Class [RN(ECs)], more commonly known as Nurse Practitioners (NPs) are graduate-prepared Registered Nurses, regulated since 1998 in the Extended Class by the College of Nurses of Ontario (CNO). NPs must meet rigorous requirements and standards to enter and maintain ongoing registration in Ontario. NPs independently, and in collaboration with health care professionals, provide health care services for all ages and across the health spectrum (e.g., primary care, acute care and long-term care). In Ontario, NPs are authorized to:
- Complete a comprehensive health history and assessment;
- Formulate and communicate a diagnosis;
- Prescribe medications;
- Dispense, sell, and compound medications;
- Order and interpret laboratory tests;
- Order diagnostic imaging tests
How do I receive services from the Health Zone NPLC?
If you are currently a patient/client of the Health Zone NPLC: call to book an appointment. The Health Zone NPLC is not a walk in clinic.
If you are not yet a patient/client of the Health Zone NPLC: Click on "New Patient Application" on the website to get an application.
Do I need an Ontario Health Card to use the services at the Health Zone NPLC?
No, you do not need a Health Card to be seen; however, if you have one, bring it with you in case you need any tests or referrals otherwise there may be an associated cost. Health Zone NPLC can help you obtain a Health Card, if needed.
Do I need to live in the neighbourhood where the Health Zone NPLC sites are located to use the services?
No, you do not have to live near the Health Zone NPLC to receive services. We offer Services to London Middlesex and surrounding areas.

What do I do if you are closed and I need help?
If it is an emergency, call 911, or go to your nearest Emergency Department or Urgent Care Centre. Please provide them with you Nurse Practitioner's name so they can send any clinic reports from your visit. If this is a mental health emergency, you can also call Reach out at 1-866-933-2023 or visit their website at Reach Out.
If it is not an emergency, call the office to book an appointment with your Nurse Practitioner.

How can I get an appointment with the clinic?
Please call ahead to book an appointment. Phones are answered Monday - Friday from 9:00am - 4:00pm (Closed 12:00pm-1:00pm for lunch)
Please note that we are not a walk-in clinic and are required to be a registered patient and book an appointment to be seen.

Please bring your Ontario health card or interim federal health papers ("brown paper"), or any medical insurance documents.

Can I come to the Health Zone NPLC if I have a family doctor?
We ONLY accept clients who do not have a family doctor or other Nurse Practitioner or if their primary care provider is retiring.
